We recognize that your privacy is very important and take it seriously, and we have prepared this Privacy Policy (this “Privacy Policy”) to provide you (the “Providers”) with important information about the privacy practices applicable to our websites, software, platforms, services, telemedicine products, and any website, application, product, or service that links to or refers to this Privacy Policy (collectively, the “Services”).

Zaidoc provides Providers with Services to facilitate medical practices, personal health records, communications, and other related activities. As a service provider for health service Providers, Zaidoc does not own or control the information submitted to us through the Services. The information submitted through the Services is governed by our agreements with Providers, including our Service Agreement and HIPAA Business Associate Agreement (collectively referred to as our 'Agreement'). In the event of any conHlict between the provisions of this Privacy Policy and our Agreement, the provisions of our Agreement will take precedence.

This Privacy Policy covers any information that, alone or in combination with other information, may be used to readily identify, contact, or locate an individual (“Personal Information”) that is in our possession. This includes, but is not limited to, Patient’s information shared by Providers, Patients, third parties at the direction of users, and Provider systems, as well as information obtained through the Services.

'Personal Information' also includes identiHiable health information collected about you. We do not consider Personal Information to include information that has been anonymized to the extent that it no longer allows a third party to easily identify a speciHic individual. This Privacy Policy describes how Zaidoc collects, uses, and discloses Personal Information.

In addition, individually identiHiable information that you provide to us for purposes of providing medical care to your potential or existing patients (“Patient” or “Patients”) (such information is also referred to as “Protected Health Information” or “PHI”) will also be subject to each Provider’s Health Insurance Portability and Accountability Act Notice of Privacy Practices (the “HIPAA Notice”), which each Provider shall make available to Patients. The HIPAA Notice shall describe how the Providers can use and share the Patients’ PHI and also describes the Patients’ rights with respect to their PHI.

This Privacy Policy does not reHlect the privacy practices of the Providers and Zaidoc is not responsible for our Providers’ privacy policies or practices. Zaidoc does not review, comment upon, or monitor a Provider’s HIPAA Notice or their compliance with their respective privacy policies, nor does Zaidoc review our client’s instructions to determine whether they are in compliance or conHlict with the terms of a client’s published privacy policy or applicable law.

We do not establish direct relationships with our Providers' patients and clients.
If we receive any inquiries or requests from our Providers' patients regarding their Personal Information, we will redirect those inquiries or requests to the relevant Provider.


If you are a client of one of our customers, we may retain your Personal Information on behalf of that customer. If you have any questions about how we handle your Personal Information, we encourage you to contact the appropriate customer. Any inquiries directly received from you concerning the use of your Personal Information will be forwarded to that customer.

We will post any modiHications or changes to this Privacy Policy on this page.

What Data we collect.


Zaidoc may collect the following kinds of information (including Personal

Information and PHI of your Patients) through our Services.

We collect information in different ways: if and when you or the Patients provide directly to us, automatically through operating our Services, and from outside sources.

Information You Or The Patients Provide Directly To Us. For certain activities, such as when the you or the Patients register, use our online services (via our websites or applications), our telemedicine services, subscribe to our alerts, or contact us directly, we may collect some or all of the following types of information:

Registration Information:

We ask for basic information from you or Patient in order to set up an account - for example, such as your and the Patients’ full name, email address, mobile phone number, and address, and unique identiHiers such as usernames and passwords.

Information about your employment, such as your job title, practice area, primary specialty, and medical license status, gender, date of birth, educational background, address, photograph, professional license information.

Transaction and Billing Information:

You will provide additional personal and payment information necessary to process the transaction and your payment, including your name, bank details, credit card information, and contact information.

Personal health information: This information includes details about the patient's diagnosis, past treatments, overall health, health insurance, and any information manually entered by you or the patients. This also includes information stored in our device (to the extent they choose to synchronize that data with our services).

Content Information:

In connection with using the Services, you may generate content that provides us with additional information about you.

Communications with Us:

You may also provide us information when you respond to surveys or communicate with us about a support question.

Information We Collect Automatically.

We also collect some information automatically:

Log Information: we collect information that web browsers, mobile devices, and servers typically make available, such as the IP address, the date and time of access. We collect log information when you use our Services.

Usage Information: We collect information about your usage of our Services. For example, we collect information about the actions that site administrators and users perform on a site–in other words, who did what, when and to what thing on a site (e.g., [username] deleted “[content]” at [time/date]). We also collect information about what happens when you use our Services along with information about your device. We use this information to, for example, provide our Services to you, as well as get insights on how people use our Services, so we can make our Services better.

Location Information:

We may determine the approximate location of your device based on your IP address. We collect and use this information, for example, to allow our customers to properly conHigure date and time-based scheduling.

Aggregated Data. We may also collect, use and share "Aggregated Data" for any purpose. Aggregated Data may be derived from your Personal Data, but once in aggregated form it will not constitute Personal Data as this data does not directly or indirectly reveal your identity. However, if we combine or connect Aggregated Data with your Personal Data so that it can directly or indirectly identify you, we treat the combined data as Personal Data which will be used in accordance with this Privacy Policy.

Children's Data.

We do not knowingly allow individuals under the age of 18 to create accounts for accessing our services. Additionally, our websites and platforms permit individuals above the age of 18, including parents and guardians like yourself, to provide, share, and store personal information about others, including minors and children. Users who provide, store, or submit information on behalf of a child assume full responsibility for the submission, use, and transmission of such information.

Zaidoc may collect non-personally identiHiable information regarding your usage of our Websites and Platforms, including the pages you viewed, time spent using the website, demographic data such as server locations, connection speed, and other information that does not identify you. Like most online services, we also use cookies and other technologies that may collect Personal Data.

What are cookies? When you visit the Services, we may send one or more "cookies" – small data Hiles – to your computer to uniquely identify your browser and let Zaidoc help you enhance your navigation through the Site. A cookie may convey anonymous information about how you browse the Services to us so we can provide you with a more personalized experience, but does not collect personal information about you. A persistent cookie remains on your computer after you close your browser so that it can be used by your browser on subsequent visits to the Service. Persistent cookies can be removed by following your web browser's directions. A session cookie is temporary and disappears after you close your browser.

What happens when you do not provide necessary Personal Data?

Where we need to process your Personal Data either to comply with law, or to perform the terms of a contract we have with you and you fail to provide that data when requested, we may not be able to perform the contract we have or are trying to enter into with you (for example, to provide you with the functionalities of the Services). In this case, we may have to stop you from using our Services.

 

Electronic Notices

By using the our Services or providing Personal Information to us, you agree that we may communicate with you electronically regarding security, privacy, and administrative issues relating to your use of the Services. If we learn of a security system’s breach, we may attempt to notify you electronically by posting a notice on the Services or sending a text or email to you.

 

Who We Share Your Personal Data With?

We may share your Personal with third parties in the ways that are described in the table below. We consider this information to be a vital part of our relationship with you.

Business Transfers.

In connection with any merger, sale of company assets, or acquisition of all or a portion of our business by another company, or in the unlikely event that Zaidoc goes out of business or enters bankruptcy, customer information would likely be one of the assets that is transferred or acquired by a third party. If any of these events were to happen, this Privacy Policy would continue to apply to your information and the party receiving your information may continue to use your information, but only consistent with this Privacy Policy.

We may share and disclose information with your consent or at your direction. For example, we may share your information with third parties with which you authorize us to do so.

We will retain your information for as long as it is reasonably needed for the purposes outlined in 'How We Use Your Information and Why'. To provide you with the best possible service, we leverage Amazon Web Services (AWS) for data storage and processing. AWS offers data centers in various regions around the world. This means the location of your data may depend on your geographic location when you use our services. We take all reasonable steps to ensure that your data is treated securely and in accordance with applicable data protection laws, regardless of where it is located.

How We Protect Your Personal Data

Zaidoc employs industry-standard physical, managerial, and technical safeguards to preserve the integrity and security of your personal information. Access to your Personal Data is restricted to employees and other staff with a business need for such access, all of whom are bound by a contractual duty of conHidentiality. We conduct periodic reviews of our policies and procedures to assess their effectiveness and ensure they remain up-to-date. Additionally, we have established procedures to address any actual or suspected

 

Personal Data breaches.

It's important to note that Zaidoc cannot guarantee the protection, control, or management of your Personal Data as outlined in this Privacy Policy if you share your login and password information with any third party, including those operating websites or providing other services.

Our services may include links to other websites not operated or controlled by Zaidoc. We are not responsible for the content, accuracy, or opinions expressed on such websites, and they are not investigated, monitored, or checked for accuracy or completeness by us.
